All admissions require the following information from hospital or physician's office:

1. History and Physical (H&P)
2. Fact Sheet (Patient demographic information)
3. Latest Lab Summary Report
4. List of Patient's Medications

Once the above information has been received, each patient is assessed on an individual basis by a Nazareth Hall Registered Nurse. Upon completion of the assessment a determination may be made for medical necessity and skilled nursing or therapy placement.
